Join our client's team as an Enrollment Registered Nurse for a PACE Center in Los Angeles. As an Enrollment Registered Nurse, you will help to bring the PACE (Program of All-Inclusive Care for the Elderly) program to serve frail elders and disabled older adults in underserved communities across Los Angeles. 

The Enrollment RN is a key player of the Outreach Team as we find and assess prospective participants for our PACE program. This person acts as a liaison between the IDT (Interdisciplinary Team) and the Outreach Team to ensure a smooth enrollment process for the prospective participants.

  • Full Time Permanent Position
  • Monday-Friday, 8:00AM-5:00PM
  • Mostly in-field home appointments and occasional onsite work in Los Angeles clinic up to 5 days per month.
  • 95% Travel to Patient Homes
  • 3-5 Visits/Day Dependent on Enrollees

RESPONSIBILITIES

  • Collaborate with Outreach Specialist and IDT to ensure successful enrollment
  • Conduct home visits to evaluate the prospective participant for potential eligibility for nursing home level of care
  • Conduct home safety evaluations of the prospective participants’ home
  • Participate in Enrollment Eligibility meetings to review Level of Care recommendations 
  • Complete Nursing Facility Level of Care (NFLOC) assessments and submit to DHCS for review; update/revise the assessment as requested by DHCS
  • Maintain accurate documentation and record all evaluations in CRM software 
  • Participate in all PACE Audits by DHCS and CMS
  • Support the Center Director to develop participant retention strategies
  • Maintain knowledge and expertise in eligibility, enrollment, and program specifications for the PACE Program
  • Support business development and census growth by participating and assisting with outreach programs as needed


REQUIREMENTS 

  • Unrestricted California Registered Nurse license 
  • Experience working in Geriatric or Senior Care
  • Passion and mission orientation for serving high-risk seniors and frail older adults 
  • In-Home experience preferred
  • Enrollment experience a plus 
  • CPR and First Aid certification (within 60 days of employment)
  • Driver’s License
