Join our client's team as an Enrollment Registered Nurse for a PACE Center in Los Angeles. As an Enrollment Registered Nurse, you will help to bring the PACE (Program of All-Inclusive Care for the Elderly) program to serve frail elders and disabled older adults in underserved communities across Los Angeles.
The Enrollment RN is a key player of the Outreach Team as we find and assess prospective participants for our PACE program. This person acts as a liaison between the IDT (Interdisciplinary Team) and the Outreach Team to ensure a smooth enrollment process for the prospective participants.
- Full Time Permanent Position
- Monday-Friday, 8:00AM-5:00PM
- Mostly in-field home appointments and occasional onsite work in Los Angeles clinic up to 5 days per month.
- 95% Travel to Patient Homes
- 3-5 Visits/Day Dependent on Enrollees

RESPONSIBILITIES
- Collaborate with Outreach Specialist and IDT to ensure successful enrollment
- Conduct home visits to evaluate the prospective participant for potential eligibility for nursing home level of care
- Conduct home safety evaluations of the prospective participants’ home
- Participate in Enrollment Eligibility meetings to review Level of Care recommendations
- Complete Nursing Facility Level of Care (NFLOC) assessments and submit to DHCS for review; update/revise the assessment as requested by DHCS
- Maintain accurate documentation and record all evaluations in CRM software
- Participate in all PACE Audits by DHCS and CMS
- Support the Center Director to develop participant retention strategies
- Maintain knowledge and expertise in eligibility, enrollment, and program specifications for the PACE Program
- Support business development and census growth by participating and assisting with outreach programs as needed
REQUIREMENTS
- Unrestricted California Registered Nurse license
- Experience working in Geriatric or Senior Care
- Passion and mission orientation for serving high-risk seniors and frail older adults 
- In-Home experience preferred
- Enrollment experience a plus
- CPR and First Aid certification (within 60 days of employment)
