JOB DETAILS

• 100% Remote but must have a New Jersey NP License
• Telehealth Provider for mostly Medicare Patients
• 8 Week Contract – April 29 – June 28
• Mon-Fri, 8AM-5PM
• Value-Based Healthcare Company
• Must be fluent in Spanish

RESPONSIBILITIES

• Quickly discern accurate patient status, conditions, and medications
• Care coordination assessments.
• Patient volume management

REQUIREMENTS 

• Valid New Jersey Nurse Practitioner (NP) with no restrictions
• Must be bilingual in English and Spanish
• Board Certified 
• DEA License
• BLS Certification
• Minimum 2 years of NP experience
• Telehealth experience preferred
• Must be able to work the full-time schedule Mon-Fri, 8AM-5PM for the duration of the contract
• Already credentialed with health plans a PLUS
