Join our client's mission-driven team as a Registered Nurse for a PACE (Program of All-Inclusive Care for the Elderly) provider.The ideal candidate will be passionate about delivering high-quality, integrated care to frail older adults and will play a critical role in the management and delivery of direct nursing care to PACE participants. RESPONSIBILITIES

  • Deliver direct nursing care to PACE participants in various settings including clinic, care center, home, virtually, and SNF
  • Conduct nursing assessments, inclusive of physical, psychosocial, and behavioral status
  • Educate participants, caregivers, and staff on how to personalize and carry out care plans and respond to situations that arise in each participant’s life
  • Monitor and evaluate therapeutic interventions, and recommend changes as needed to proactively improve health and wellbeing

 

REQUIREMENTS 

  • Unrestricted California Registered Nurse license 
  • Graduate of accredited RN program; BSN strongly preferred
  • CPR and First Aid certification (within 60 days of employment)
  • Alzheimer's certification
  • Trained in wound care, IVs, blood draws
  • Passion and mission orientation for serving high-risk seniors and frail older adults 
  • Minimum 2 years of experience caring for adults, chronically disabled or frail, or elderly populations; Ambulatory care experience is helpful but not essential
  • Ability to function independently in the day-to-day management of the clinical needs of participant and the operations of the PACE Center Clinic utilizing the company’s policies and procedures, leadership guidance and program goals in the decision-making process
  • Active listening and conflict management skills 
  • Ability to adapt quickly to multiple clinical settings
  • Must be fluent in Spanish 
  • Ability to lift and carry or otherwise move up to 50 pounds 
  • Ability to assist Participants with mobility challenges
