Join our client's team as an LVN - Home Care Coordinator to develop and implement home care services for PACE participants in Los Angeles, California. The ideal candidate will have a passion for serving high-risk seniors and frail older adults, possess an LVN License, and have experience in home care services management.
For this role, you will also perform the coordination of all Durable Medical Equipment and community-based care. You are a collaborative team member who works as part of the Interdisciplinary Team to create a care plan to help participants’ meet their goals. This is primarily a center-based role.
- Full Time Permanent Position
- On-Site at a PACE Center close to downtown Los Angele
- Monday-Friday, 40 Hours a Week

RESPONSIBILITIES
- Coordinate home care services for PACE program participants
- Participate as a member of the Interdisciplinary Team (IDT)
- Conduct nursing care within LVN Scope of practice.
- Provide general nursing care in the clinic and participant’s home
- Document in the EMR within set documentation timeline.
- Communicate with home care employees to understand participant’s condition
- Collaborate with the Transportation Coordinator for participant transportation
- Schedule Home Care appointments and support the scheduler for other appointment needs
- Oversee visits completed by external agencies.
REQUIREMENTS
- Unrestricted California Licensed Vocational Nurse (LVN) license
- Must have Home Care and Geriatric experience
- Home Care coordination a plus
- Fluent in Spanish
- CPR and First Aid certification (within 60 days of employment)
