Join our client's mission-driven team as a Personal Care Attendant for the PACE Program. As a Personal Care Attendant at our client, you will provide outstanding care to frail and vulnerable older adults in the community. You will interact in a friendly, courteous and professional manner with participants and their family/caregivers, providing personal care and escort assistance in alignment with participants’ treatment plans, and collaborating with the interdisciplinary care team to monitor for changes in participants’ conditions.

RESPONSIBILITIES
- Supports participants with activities of daily living in the clinic and center, and occasionally the home. Support may include escorting, toileting, bathing, grooming, laundry and assistance with meals
- Assists participants with entering and exiting transportation vehicles and acting as a medical escort when instructed
- Escorts participants to offsite medical appointments
- Conducts daily physical activities in assisting frail elders in ergonomic manner to avoid injury
- Assists with food service delivery and monitoring during mealtimes
- Support participants during mealtime by ensuring participants receive the correct meals and aiding participants in feeding as needed
- Maintains personal care supplies such as incontinence supplies and personal grooming items
- Carries out orders from the interdisciplinary team regarding personal care services and/or monitoring of participant status
- Communicates effectively to participants, families, staff and other agencies
REQUIREMENTS
- Certified Nursing Assistant (CNA) certification or Home Health Aide (HHA) certification preferred but not required
- Bilingual in English/Korean required
- CPR and First Aid certification (within 60 days of employment)
- Passion and mission orientation for serving high-risk seniors and frail older adults
- Experience as a personal care attendant or certified nursing assistant
- Experience with the frail and elderly population
- Proof of a valid CA driver’s license, personal transportation, good driving record and auto insurance as required by State law
- Flexibility to perform role in the home on an as needed basis
