JOB SUMMARY

The case manager performs the primary functions of assessment, planning, facilitation and advocacy, which are achieved through collaboration with the client and other health care professionals involved in the client's care.

REQUIREMENTS 

• Active and unrestricted California Registered Nursing (RN) License
• Bachelor’s Degree in Nursing. Master’s Degree in Healthcare or related field preferred
• 2 years of experience in Case Management (Care Coordination or Utilization Management) in an acute care hospital
• 1 year of supervisory experience
• Computer proficiency
