• Monday – Friday, 8 Hours a Day (Set Schedule and No Weekends)
• This role is mostly virtual calls/appointments with families and their children (no long hours in a high stress environment)

JOB SUMMARY

As a Social Worker, you will work with the families of medically complex children providing emotional support, care coordination, connection to resources, and support navigating the healthcare system. You leverage an integrated technology platform and are complimented by an entire interdisciplinary team including MDs, APPs, RNs, navigators, pharmacists, and dietitians.

RESPONSIBILITIES

• Complete psychosocial assessment and assists team with the development of the plan of care, integrating patient/family preferences and values 
• Plan, coordinate, manage and implement support packages to help Imagine families deal with difficulties related to home environment, health literacy, housing instability, food insecurity, etc. 
• Advocate for resources and removal of barriers and connect families with available resources 
• Participate in ongoing scheduled consultations with an interdisciplinary team to monitor patient progress
• Represent the company commendably to patients, families, providers, and community

REQUIREMENTS

• Master's degree in Social Work with at least 12 months clinical experience. 
• Texas Licensure as a Licensed Master Social Worker (LMSW)/Licensed Clinical Social Worker (LCSW) Licensed Professional Clinical Counselor (LPCC)
• Experience with team-based care and with excellent collaboration skills. 
• Excellent working knowledge of community resources. 
• Experience working with a pediatric population strongly preferred, 
• Virtual care experience a plus. 
• Bilingual Spanish strongly preferred
• Proof of complete COVID vaccination
