• Hours vary depending on the number of visits per week required for each patient. Typically each patient is required to have 1-3 visits per week

JOB SUMMARY

In this role the Certified Home Health Aide (CHHA) will receive referrals from hospitals for Medicare Patients. Once they receive a referral (assigned by the Director of Patient Care Services), they will be responsible for traveling to the patients home to provide personal care and assist the patient in activities of daily living under direction of a registered nurse, physical therapist, speech therapist, or occupational therapist. 

The Home Health Aide assignment shall be made in accordance with the plan of treatment. Each case goes on for 9 weeks (2mo) per patient. The RN will determine how many times per week (could be 1-3 times per week) the patient is to be visited and this will be the CHHA’s schedule. After the 9 weeks (2mo) is completed, the patient will be reevaluated and additional/necessary changes to the Plan of Care or Plan of Treatment will be made if an additional 9 weeks are needed to be assigned to the CHHA.

Visits last 45 mins-1 hr each

REQUIREMENTS

• Active and valid license as a CA Certified Home Health Aide (CHHA)
• CPR Certification
• Valid Driver’s License with proof of insurance
• Reliable mode of transportation
• Able to lift up to 40 lbs on a frequent basis
• Able to stand or sit for 6-8 hours a day.
• Excellent customer service skills
