JOB SUMMARY

Nurse Practitioner panels are around 100 - 200 patients. You will be seeing 2-4 patients a day when in the field. 

The assigned location/territory is within-a 2 Hour DriveTravel to patient homes is considered part of the workday. So, depending on travel times, you may only have 1 or 2 patients per day but compensated according to full-time salary.

 

RESPONSIBILITIES

  • Deliver evidence-based, timely care in a manner that reduces avoidable hospitalizations, maximizes quality of life, and puts patient health and satisfaction first.
  • Conduct assessments, which includes comprehensive annual wellness exams on patients. (Assessments will occur in-home.) 
  • Prescribe medications, order tests, and collaborate with patient’s physician. 
  • Document items such as: chief complaint, all applicable diagnosis, past medical, family, and social history, review of systems, examinations, medications, allergies, assessment, and plan.
  • Collaborate with care management RNs and SWs to ensure consistent, highly satisfying patient experience, accurate documentation, and effective care planning. 6. Complete all documentation and paperwork in a timely manner.
  • Maintain quality of care and productivity standards as defined by the practice. 
  • Perform effectively, as reflected by improved patient quality outcomes, which will be measured and reported daily.

 

REQUIREMENTS 

  • Active and unrestricted Texas Registered Nurse Practitioner license
  • 2+ years experience in Home Health Care and or Hospice
  • 2+ years experience with Managed Care 
  • Experience with computer-based data management programs
  • Able to show proof of complete COVID vaccination
  • Preferably with experience conducting annual wellness visits or similar comprehensive visits and appropriately charting HCCs and has general knowledge of risk adjustment.
  • Ability to drive to patients’ homes
