- Location: South Bay, Los Angeles, San Fernando Valley, Antelope Valley
- Work/Life Balance (4 x 10hr days per week, No Saturdays, no rotating Schedule – Only 4 Patients per day)
- 8:00AM – 6:30PM
- Minimal Travel –will work in one zip code and only that zip code
- Provided all equipment for daily meetings with Patients/Providers (iPad, SmartPhone)
- No In-house Visits are done until Covid Ends – Patients will be prepared outside for visit

JOB SUMMARY
Licensed Vocational Nurses perform in-home care as part of a proactive, multi-disciplinary approach to managing complex, chronically ill, and palliative care patients. This role is ideal for a passionate clinician who wants to shape the future of healthcare delivery for complex medical and social needs in a fast-paced environment.
RESPONSIBILITIES
- Assess patients’ condition during every visit and chart observations in electronic medical record
- Perform evaluation tasks, including vital signs, data collection with medical history intake, and medication reconciliation; reporting findings to the patient’s provider
- Assists outreach team engaging patients in the community
- Coordinate with multidisciplinary teams necessary Occupational Therapists, Physical Therapists, Physicians and anyone else involved in the patient’s care plan
- Provide palliative care as needed to keep the patient comfortable
- Make recommendations for devices or tools that might improve the patient’s quality of life
- Actively listen to the patient and respond to concerns or requests
- Ability to travel to multiple patients’ homes per day
- Perform venipuncture & finger sticks to obtain blood samples.
- Prepare specimens & specimens for lab analysis.
- Educate patients on general health & basic self-care procedures.
- Review, explain & clarify physician orders with patient
- Provide education and instruct patient in use of home equipment
- Actively participates in panel management bi-weekly clinical rounds
- Patient care may be performed in any setting, including, but not limited to, shelter, transitional housing, assisted care and living facility, the patient's home or mutually designated location with privacy
- The direction provided by a registered nurse, nurse practitioner, or physician to the LVN is (must/will be) available by remotely linking/connecting and at least by telephone
- Assist outreach as needed, locating patients, providing explanation of services, helping to enroll difficult to reach patients.
- Perform other duties as directed
REQUIREMENTS
- 1+ years of experience in an outpatient or home health setting
- Active California LVN License
- BLS Certified
- Active Driver’s License
- IV and Phlebotomy Certified (Required – If they do not have this, they are required to obtain it within 30 days of being hired)
- Excellent communication and time management skills
- Must be compassionate and very comfortable visiting patients in their home or elsewhere
- Creative, organized and flexible with a can-do attitude
- Ability to communicate in Spanish a plus
