What You’ll Do:

  • Conduct in-home evaluations focused on safety, mobility, and function

  • Create personalized care plans that support independence

  • Recommend adaptive equipment and environmental modifications

  • Collaborate with nurses, PTs, social workers, and families

  • Document clearly with an intuitive EMR (we value your time)

What You’ll Need:

  • Active Arizona OT license

  • Reliable transportation and valid driver’s license

  • Home health experience preferred, but we welcome strong clinicians from any setting

  • Strong communication and time management skills

  • A heart for seniors and a passion for person-centered care
