What You’ll Do:
Conduct in-home evaluations focused on safety, mobility, and function
Create personalized care plans that support independence
Recommend adaptive equipment and environmental modifications
Collaborate with nurses, PTs, social workers, and families
Document clearly with an intuitive EMR (we value your time)
What You’ll Need:
Active Arizona OT license
Reliable transportation and valid driver’s license
Home health experience preferred, but we welcome strong clinicians from any setting
Strong communication and time management skills
