This full-time position is specifically designed for individuals passionate about providing top-tier health and palliative care management to patients in their residences. It's essential to note that this job is not a remote position; our intake team collaborates closely at our facilities to ensure that every patient receives personalized and effective care planning and support from the moment they begin their services with us.

As an integral component of our home health and hospice team, the successful candidate will play a crucial role in the first phase of patient entry, using their in-depth knowledge and understanding of healthcare protocols to assess and facilitate the necessary services. This role requires a detail-oriented approach, with a strong emphasis on accuracy and empathy, understanding the delicate nature of well-being and the essential communication involved with patients and families beginning potentially life-altering care processes.

Duties and Responsibilities

  • Conduct initial case evaluations to determine appropriateness of care, including establishing eligibility and service needs.
  • Manage and process referrals from physicians, hospitals, and other healthcare providers, ensuring seamless transition into home health or hospice care.
  • Provide detailed and clear information to patients and their families regarding the specifics of the service, care plans, and expectations.
  • Coordinate with healthcare professionals to plan and set up initial care and service provisions.
  • Maintain comprehensive records of all interactions, assessments, and plans in the patient's health care file.
  • Stay abreast of the latest developments, regulations, and compliance requirements in home health and hospice care.
  • Review patient intake forms for completion and accuracy, and liaise with insurance providers to confirm coverage and benefits.
  • Participate in regular team meetings to discuss patient needs, issues, and updates.
  • Ensure all patient data is entered into system databases promptly and accurately to assist in ongoing care coordination.
  • Support the clinical team by providing them with insightful data concerning patients' health conditions and potential needs.
  • Adapting the intake process based on evolving trends and feedback from healthcare professionals and clients to improve service delivery.
  • Commit to the ethical considerations inherent in hospice and home health work, ensuring all patient interactions are conducted with dignity and respect.

Requirements

  • Bachelor’s degree in Nursing, Healthcare Administration, or a related field.
  • Valid Registered Nurse (RN) or Licensed Practical Nurse (LPN) license preferred.
  • Minimum of three years’ experience in a healthcare setting, with direct experience in home health or hospice care settings strongly preferred.
  • Proficient understanding of medical terminology, healthcare protocols, and compliance requirements in home health and hospice care.
  • Excellent communication skills, both verbal and written, for effective interaction with a diverse group of patients, healthcare professionals, and family members.
  • Strong organizational skills and the ability to manage multiple tasks simultaneously without compromising the quality of work.
  • Exceptional interpersonal skills, with a proven ability to show empathy and tact in sensitive situations.
  • Experience with healthcare information systems and proficiency with electronic medical record software.
  • Meticulous attention to detail and a commitment to providing accurate and timely information.
  • Demonstrated ability to maintain patient confidentiality following HIPAA guidelines.
  • A proactive approach to problem-solving and the capacity to make informed decisions quickly.
