The Nurse Case Manager will be responsible for a comprehensive range of activities aimed at optimizing patient care while ensuring effective use of resources. This role demands a high level of clinical expertise, excellent communication skills, and the ability to coordinate with various healthcare professionals to plan and implement effective patient care strategies.
Duties and Responsibilities
- Conduct comprehensive assessments of patient health needs and resources.
- Develop, implement, and regularly update individualized care plans based on patient assessments.
- Coordinate with physicians, specialists, nutritionists, and other healthcare professionals to ensure appropriate referrals, follow-up appointments, and optimal patient care.
- Provide direct patient care as needed, complementing the care delivered by other team members.
- Monitor patient progress and outcomes, reassessing and modifying care plans accordingly.
- Educate patients and their families on health conditions, treatment plans, and any aspects of health maintenance and prevention.
- Advocate for patients’ needs and desires with respect to health care, working with management and policy makers to improve access and effectiveness.
- Maintain complete and accurate records of all interactions and communications with patients and healthcare providers.
- Manage a caseload of patients, demonstrating excellent time management and organizational skills to prioritize needs efficiently.
- Ensure compliance with all health care laws, standards, and regulations.
- Participate in multidisciplinary teams to review and optimize patient care, particularly for those with complex health conditions.
- Lead workshops and informational sessions for staff development on case management practices and standards.
- Utilize technology and health care informatics to enhance patient care outcomes.
Requirements
- Registered Nurse (RN) License required.
- Bachelor’s degree in Nursing or related field (Master’s preferred).
- Minimum of 3 years’ experience in a clinical nursing position.
- Prior experience as a Home Healthcare RN Case Manager strongly preferred.
- Demonstrated ability to work compassionately and effectively with diverse patient populations.
- Strong clinical skills with the ability to work both independently and as part of a team.
- Excellent communication and interpersonal skills, capable of navigating complex conversations with patients and families.
- Proficiency in medical software and patient management systems.
- Knowledge of healthcare regulations and case management standards.
- Commitment to ongoing professional development and continuing education.
- Proven ability to manage multiple tasks and priorities in a high-paced, dynamic environment.
