Job Description Summary:

The Clinical Coordinator provides essential support to clinical administration, patients, families, caregivers, co-workers, and other healthcare team members by assessing, planning, and evaluating patient situations to ensure quality patient care. The coordinator supervises all patient care activities while reporting to the Home Health Clinical Supervisors.

Major Duties and Responsibilities:

  • Assist Clinical Supervisors in assigning staff to new referrals based upon patient needs, staff skills, and geographic location. Communicate all relevant information regarding referral to staff.
  • Maintain accurate field staff schedules. Update schedules daily based upon new assignments and communication from field staff. Submit reports to Managers,
  • Clinical Supervisors and Accounting.
  • Maintains weekend, holiday and vacation schedules. Assist Clinical Supervisors in completing weekend and holiday assignments and arranges vacation coverage for field staff.
  • Data Entry of field staff assignments.
  • Participates in quality improvement activities.
  • Assist with orientation and training of other staff members.

Knowledge, Skills and Abilities:

  • Prefer clinical certification such as LPN, MA, or equivalent and a minimum of one year’s clinical experience. Home care experience strongly preferred.
  • Excellent communication and organizational skills. Knowledge of office machines required.
  • Employee is required to sit, stand and walk frequently; and to lift up to 25 pounds occasionally.
  • Job requires occasional bending, kneeling, reaching forward and overhead, and lifting floor to waist and waist to overhead. Position involves frequent CRT terminal work, requiring continuous sitting and use of both hands for grasping, pushing/pulling and fine manipulation; driving requires use of at least one foot.
