About the Role
We are seeking a highly skilled and compassionate Nurse Practitioner to support our growing programs. This part-time role offers flexible hours and will transition into a full-time position as we continue to expand. In this role, you will work closely with leading local physicians to provide holistic, patient-centered care in a home-based setting.

Key Responsibilities:
✔ Conduct comprehensive patient assessments and develop personalized care plans
✔ Deliver in-home medical oversight in collaboration with interdisciplinary teams
✔ Provide care in alignment with primary care, palliative care, or wound care programs
✔ Educate patients and families on health conditions, treatment plans, and preventive care
✔ Document patient progress and ensure compliance with healthcare regulations
✔ Participate in the development of innovative care models

Qualifications:
✔ Active Nurse Practitioner license in Texas & New Mexico (or eligibility to obtain)
✔ Experience in home-based care (wound care, palliative care, or primary care preferred)
✔ Strong interpersonal and team leadership abilities
✔ Passion for delivering high-quality, compassionate care
✔ Ability to work independently and collaboratively in a home-based setting
