Salary Range:  $95000 - $115000 year

Sterling Care, a locally owned and operated 5-star CMS-certified home health agency based in Greenwich, Connecticut, is currently seeking a full-time Physical Therapist to perform home physical therapy visits for our patients in lower Fairfield County. New graduates and experienced therapists are welcome to apply. Enjoy the fulfillment of patient-centered, one-on-one care in the comfort of patients' homes. We help our patients recover after an illness, manage chronic conditions, or adjust to new limitations by designing appropriate care plans with goals specific to their home environment. 

Compensation: $95,00 to $115,000 plus benefits depending upon your education, experience, and workload.

Physical Therapist Responsibilities:

- Evaluate and assess patients' physical conditions, medical history, and goals.

- Develop personalized treatment plans based on the patient's needs and goals.

- Implement therapeutic exercises, manual therapy techniques, and other modalities to improve patients' mobility, strength, and function.

- Monitor and document patients' progress and adjust treatment plans as necessary.

- Educate patients and their families on exercises, self-care techniques, and injury prevention strategies.

- Collaborate with other healthcare professionals to coordinate patient care and ensure optimal outcomes.

- Stay updated on the latest research and advancements in physical therapy techniques

Physical Therapist Qualifications:

- Graduate of an accredited physical therapy program.

- Valid physical therapy license.

- Solid computer skills. Driver's license.Must be able to work in the United States.

- Must have a car in good working condition.
