Job Description

The Cow Creek Band of the Umpqua Tribe of Indians, Social Services department, is excited to announce an opportunity for a dedicated and seasoned professional to join our team as a Self-Sufficiency Case Manager. This full-time position plays a vital role in supporting and empowering our community members through various self-sufficiency programs. The selected candidate will be centrally involved in managing and implementing initiatives aimed at enhancing the living standards and overall well-being of individuals and families within our tribal community.

This position is not remote and requires the individual to be on-site, providing direct support and guidance to our people. The role requires a hands-on approach to address the multifaceted challenges faced by our community members, aligning with our commitment to uplift and provide high-quality growth opportunities for each member. Duties and Responsibilities

  • Maintain participant and program records and waiting lists, including file maintenance and data entry.
  • Assist in calculating and recalculating family income for income-based programs.
  • Schedule new participant orientations, upon availability of units and request by the Social Services Director.
  • Schedule and conduct weekly, monthly, quarterly, and yearly inspections and re-certification for program applicants and participants.
  • Develop and maintain a comprehensive network of education, training, economic development, and other supportive services for youth and adults.
  • Work with participants to reach self-sufficiency goals by conducting assessments, intakes, and creating plans that are clear, measurable, realistic, and timely.
  • Complete well-organized case files that reflect services provided and outcomes accurately.
  • Complete paperwork, monthly reports, and case notes accurately and promptly.
  • Attend and actively participate in case staffing and agency meetings, as required.
  • Make and follow up on referrals, ensuring adequate and appropriate services are provided.
  • Collect program data to track participant progress every month.
  • Develop and promote self-reliance activities for families, including Employment, Financial Counseling, Permanent Housing, Home ownership, and Education.
  • Research program expectations regularly to ensure effectiveness and efficiency; develop and revise policies, procedures, and protocols as necessary.
  • Coordinate supportive services and work closely with Tribal, State, and community resources.
  • Maintain communication with ODHS Self Sufficiency regarding common clients and assist with the application process for programs such as SNAP, ERDC, TANF, Medical, and DV assistance.
  • Evaluate needs and gaps in services and work to identify solutions and provide an assessment of needs.
  • Manage Riverside Transitional Housing, including application processes, monthly inspections, bill payments, maintenance scheduling, rent collection, security monitoring, and handling violations and evictions.

Requirements

  • Associate’s Degree in Human Services or related field required.
  • Excellent communication skills, including verbal and written.
  • Ability to maintain confidentiality at all times.
  • Case management experience preferred.
  • Demonstrate good organizational, math, and writing skills.
  • Must have working knowledge of Microsoft Office products.
  • Current, valid, unrestricted Oregon driver’s license.
