POSITION PURPOSE:

We are looking for a dedicated and compassionate VOCA Transitional Case Manager to join our team in Social Services. You will work closely with clients to establish trust and build meaningful relationships, helping them access vital resources and develop coping strategies. Your expertise in case management will empower clients to set realistic goals, advocate for their needs, and foster self-sufficiency. As a VOCA Transitional Case Manager, you will collaborate with community organizations, mental health professionals, and service providers, ensuring that our clients receive comprehensive care tailored to their unique situations. Position Duties:  

  • Conduct comprehensive assessments to identify client needs and strengths.
  • Develop individualized service plans in collaboration with clients.
  • Provide emotional support and guidance throughout the transition process. 
  • Manage grant and work within the perimeters of the grant by meeting reporting requirements and providing report in a timely manner.
  • Connect clients to relevant community resources and services.
  • Monitor client progress and adjust service plans as needed.
  • Advocate for clients' rights and access to necessary services.
  • Maintain accurate case records and documentation of client interactions.

 

QUALIFICATIONS:

  • Bachelor's degree in Social Work, Psychology, or a related field.
  • Previous experience in case management or working with trauma survivors.
  • Working knowledge of trauma-informed care.
  • Strong understanding of resources available in the community.
  • Excellent interpersonal and communication skills.
  • Ability to work independently as well as part of a team.
  • Proficient in using case management software and Microsoft Office.
  • A passion for helping others and a commitment to client-centered care.
